mm, slab: remove unnecessary unlikely()

WARN_ON() already contains an unlikely(), so it's not necessary to use
unlikely.

Also change WARN_ON() back to WARN_ON_ONCE() to avoid potentially
spamming dmesg with user-triggerable large allocations.
