fb2a27e743 [MIPS] Reimplement clear_page/copy_page
Fold the SB-1 specific implementation of clear_page/copy_page in the
generic version, and rewrite that one in tlbex style. The immediate
benefits:
  - It converts the compile-time workaround for SB-1 pass 1 prefetches
    to a more efficient run-time check.
  - It allows adjustment of loop unfolling, which helps to reduce the
    number of redundant cdex cache ops.
  - It fixes some esoteric cornercases (the cache line length calculations
    can go wrong, and support for 64k pages without prefetch instructions
    will overflow the addiu immediate).
  - Somewhat better guesses of "good" prefetch values.

bf2ae2b37c pageflags: standardize comment inclusion in asm-offsets.h and fix MIPS
Add the ability to pass comments into asm-offsets.h by generating asm
output like

-># comment line

Mips needs this feature to preserve the comments that are in
asm-mips/asm-offsets.h right now.

Then remove the special handling for mips from Kbuild and convert mips to use
the new string to include the comments.

950e4da324 arch: Remove unnecessary inclusions of asm/semaphore.h
None of these files use any of the functionality promised by
asm/semaphore.h.  It's possible that they rely on it dragging in some
unrelated header file, but I can't build all these files, so we'll have
fix any build failures as they come up.

64ac24e738 Generic semaphore implementation
Semaphores are no longer performance-critical, so a generic C
implementation is better for maintainability, debuggability and
extensibility.  Thanks to Peter Zijlstra for fixing the lockdep
warning.  Thanks to Harvey Harrison for pointing out that the
unlikely() was unnecessary.

b4dcaea36b Pb1200/DBAu1200: fix bad IDE resource size
The header files for the Pb1200/DBAu1200 boards have wrong definition for the
IDE interface's decoded range length -- it should be 512 bytes according to
what the IDE driver does.  In addition, the IDE platform device claims 1 byte
too many for its memory resource -- fix the platform code and the IDE driver
in accordance.

AFAICT pm_send_all is a nop when noone uses pm_register...

Hmm.. can we just force CONFIG_PM_LEGACY=n, and see what happens?

Or maybe this is better idea? It may break build somewhere, but it
should be easy to fix... (it builds here, i386 and x86-64).

758e285fac [MIPS] Alchemy: work around clock misdetection on early Au1000
Work around the CPU clock miscalculation on Au1000DA/HA/HB due the
sys_cpupll register being write-only, i.e. actually do what the comment
before cal_r4off() function advertised for years but the code failed at.
This is achieved by just giving user a chance to define the clock
explicitly  in the board config. via CONFIG_SOC_AU1000_FREQUENCY option,
defaulting to 396 MHz if the option is not given...

The patch is based on the AMD's big unpublished patch, the issue seems to
be an undocumented errata (or feature :-)...

cf7b7e0b37 [MIPS] BCM1480: Fix PCI/HT IO access
- removed check for enable HT-PCI bridges, because some CFE version
  init only the needed one and scanning works even with disabled HT
  links
- implemented I/O access behind HT PCI busses

0f87358587 [MIPS] Allow 48Hz to be selected if CONFIG_SYS_SUPPORTS_ARBIT_HZ is set.
This allows a 48Hz clock to be selected on Malta and other systems.  Note
this not normally a sensible option as it results in rather high latencies
for some kernel stuff.

1f5826bd0e [MIPS] Added missing cases for rdhwr emulation
Some of these are architecturally required for R2 processors so lets try
to be bit closer to the real thing.  This also provides access to the
CPU cycle timer, even on multiprocessors.  In that aspect its currently
bug compatible to what would happen on a R2-based SMP.

6783fe6253 [MIPS] Wire up the timerfd_*() o32 system calls
This patch enables the system calls timerfd_create(), timerfd_settime()
and timerfd_gettime() for MIPS architecture.

9a74b3eb22 [MIPS] Fix buggy invocations of kmap_coherent()
kmap_coherent will only work correctly if the page it is called on is
not marked dirty.  If it's dirty the kernel address of the page should
be used instead of a temporary mapping.

2eaa7ec286 [MIPS] Handle I-cache coherency in flush_cache_range()
So far flush_cache_range() did't consider the I-cache largely because it
did rarely ever matter to real world code.  This was working primarily
because normally code and data are don't share the same pages - with the
exception of MIPS16 code which uses address constants embedded between
the code.   The following sequence of events may break the code:

 o MIPS16 executable being loaded
 o dynamic linker relocates the address constants embedded into the code:
 o   Uses mprotect(2) to make code pages PROT_READ|PROT_WRITE
 o   Performs the actual relocations by writing to the pages which likely
     are COW.  Because no PROT_EXEC is set I-cache coherence will not be
     considered.
 o   Uses mprotect(2) to switch code pages back to PROT_READ|PROT_EXEC.
     This results in a call to flush_cache_range() which also does not
     consider I-caches.
 o => executing the page just having been relocated may now result in the
   I-cache getting refilled with stale data from memory.

4ac9137858 Embed a struct path into struct nameidata instead of nd->{dentry,mnt}
This is the central patch of a cleanup series. In most cases there is no good
reason why someone would want to use a dentry for itself. This series reflects
that fact and embeds a struct path into nameidata.

Together with the other patches of this series
- it enforced the correct order of getting/releasing the reference count on
  <dentry,vfsmount> pairs
- it prepares the VFS for stacking support since it is essential to have a
  struct path in every place where the stack can be traversed
- it reduces the overall code size:

72a7fe3967 Introduce flags for reserve_bootmem()
This patchset adds a flags variable to reserve_bootmem() and uses the
BOOTMEM_EXCLUSIVE flag in crashkernel reservation code to detect collisions
between crashkernel area and already used memory.

This patch:

Change the reserve_bootmem() function to accept a new flag BOOTMEM_EXCLUSIVE.
If that flag is set, the function returns with -EBUSY if the memory already
has been reserved in the past.  This is to avoid conflicts.

Because that code runs before SMP initialisation, there's no race condition
inside reserve_bootmem_core().

6c81c32f96 calibrate_delay() must be __cpuinit
calibrate_delay() must be __cpuinit, not __{dev,}init.

I've verified that this is correct for all users.

While doing the latter, I also did the following cleanups:
- remove pointless additional prototypes in C files
- ensure all users #include <linux/delay.h>

This fixes the following section mismatches with CONFIG_HOTPLUG=n,
CONFIG_HOTPLUG_CPU=y:

9cfe015aa4 get rid of NR_OPEN and introduce a sysctl_nr_open
NR_OPEN (historically set to 1024*1024) actually forbids processes to open
more than 1024*1024 handles.

Unfortunatly some production servers hit the not so 'ridiculously high
value' of 1024*1024 file descriptors per process.

Changing NR_OPEN is not considered safe because of vmalloc space potential
exhaust.

This patch introduces a new sysctl (/proc/sys/fs/nr_open) wich defaults to
1024*1024, so that admins can decide to change this limit if their workload
needs it.

4d672e7ac7 timerfd: new timerfd API
This is the new timerfd API as it is implemented by the following patch:

int timerfd_create(int clockid, int flags);
int timerfd_settime(int ufd, int flags,
		    const struct itimerspec *utmr,
		    struct itimerspec *otmr);
int timerfd_gettime(int ufd, struct itimerspec *otmr);

The timerfd_create() API creates an un-programmed timerfd fd.  The "clockid"
parameter can be either CLOCK_MONOTONIC or CLOCK_REALTIME.

The timerfd_settime() API give new settings by the timerfd fd, by optionally
retrieving the previous expiration time (in case the "otmr" parameter is not
NULL).

The time value specified in "utmr" is absolute, if the TFD_TIMER_ABSTIME bit
is set in the "flags" parameter.  Otherwise it's a relative time.

The timerfd_gettime() API returns the next expiration time of the timer, or
{0, 0} if the timerfd has not been set yet.

Like the previous timerfd API implementation, read(2) and poll(2) are
supported (with the same interface).  Here's a simple test program I used to
exercise the new timerfd APIs:

On the per-architecture side, I do think it would be better to *not* have
internal architecture knowledge in a generic file, and as such a line like

        depends on X86_32 || IA64 || PPC || S390 || SPARC64 || X86_64 || AVR32

really shouldn't exist in a file like kernel/Kconfig.instrumentation.

It would be much better to do

        depends on ARCH_SUPPORTS_KPROBES

in that generic file, and then architectures that do support it would just
have a

        bool ARCH_SUPPORTS_KPROBES
                default y

in *their* architecture files. That would seem to be much more logical,
and is readable both for arch maintainers *and* for people who have no
clue - and don't care - about which architecture is supposed to support
which interface...

Actually, I know I gave this as the magic incantation, but now that I see
it, I realize that I should have told you to just use

        config ARCH_SUPPORTS_KPROBES
                def_bool y

instead, which is a bit denser.

We seem to use both kinds of syntax for these things, but this is really

e30ec4525d [MIPS] Split the micro-assembler from tlbex.c.
This patch moves the micro-assembler in a separate implementation, as
it is useful for further run-time optimizations. The only change in
behaviour is cutting down printk noise at kernel startup time.

Checkpatch complains about macro parameters which aren't protected by
parentheses. I believe this is a flaw in checkpatch, the paste operator
used in those macros won't work with parenthesised parameters.

a055917e1b [MIPS] Fix build after b0rked changes to <linux/init.h>.
Commit 312b1485fb made __INIT_REFOK expand
into .section .section ".ref.text", "ax".  Since the assembler doesn't
tolerate stuttering in the source that broke all MIPS builds.

Since with this change Sam downgraded __INIT_REFOK the best fix is to
get replace it by the modern days operator.  With MIPS the only user
of __INIT_REFOK and __INITDATA_REFOK (which was equally broken) being
unused anyway these can be deleted but that's subject of a separate
commit.

95c354fe9f spinlock: lockbreak cleanup
The break_lock data structure and code for spinlocks is quite nasty.
Not only does it double the size of a spinlock but it changes locking to
a potentially less optimal trylock.

Put all of that under CONFIG_GENERIC_LOCKBREAK, and introduce a
__raw_spin_is_contended that uses the lock data itself to determine whether
there are waiters on the lock, to be used if CONFIG_GENERIC_LOCKBREAK is
not set.

Rename need_lockbreak to spin_needbreak, make it use spin_is_contended to
decouple it from the spinlock implementation, and make it typesafe (rwlocks
do not have any need_lockbreak sites -- why do they even get bloated up
with that break_lock then?).

8216d348e3 [MIPS] Malta: use the KERN_ facility level in printk()
This patch adds the KERN_ macros to printk() calls. Where applicable,
spaces are replaced by tabs.

These changes noticeably reduce the number of errors and warnings
reported by the checkpatch.pl script for the malta_int.c file.

Before the patch: total: 47 errors, 20 warnings, 354 lines checked

After the patch: total: 34 errors, 7 warnings, 355 lines checked

302922e5f6 [MIPS] Qemu: Remove platform.
The Qemu platform was originally implemented to have an easily supportable
platform until Qemu reaches a state where it emulates a real world system.
Since the latest release Qemu is capable of emulating the MIPSsim and
Malta platforms, so this goal has been reached.  The Qemu plaform is also
rather underfeatured so less useful than a Malta emulation.

619b6e18fc [MIPS] R4000/R4400 daddiu erratum workaround
This complements the generic R4000/R4400 errata workaround code and adds 
bits for the daddiu problem.  In most places it just modifies handwritten 
assembly code so that the assembler is allowed to use a temporary register 
as daddiu may now be treated as a macro that expands to a sequence of li 
and daddu.  It is the AT register or, where AT is unavailable or used 
explicitly for another purpose, an explicitly-named register is selected, 
using the .set at=<reg> feature added recently to gas.  This feature is 
only used if CONFIG_CPU_DADDI_WORKAROUNDS has been set, so if the 
workaround remains disabled, the required version of binutils stays 
unchanged.

 Similarly, daddiu instructions put in branch delay slots in noreorder 
fragments are now taken out of them and the assembler is allowed to 
reorder them itself as possible (which it does making the whole idea of 
scheduling them into delay slots manually questionable).

 Also in the very few places where such a simple conversion was not 
possible, a handcoded longer sequence is implemented.

 Other than that there are changes to code responsible for building the 
TLB fault and page clear/copy handlers to avoid daddiu as appropriate.  
These are only effective if the erratum is verified to be present at the 
run time.

 Finally there is a trivial update to __delay(), because it uses daddiu in 
a branch delay slot.

20d60d9973 [MIPS] R4000/R4400 errata workarounds
This is the gereric part of R4000/R4400 errata workarounds.  They include 
compiler and assembler support as well as some source code modifications 
to address the problems with some combinations of multiply/divide+shift 
instructions as well as the daddi and daddiu instructions.

 Changes included are as follows:

1. New Kconfig options to select workarounds by platforms as necessary.

2. Arch top-level Makefile to pass necessary options to the compiler; also 
   incompatible configurations are detected (-mno-sym32 unsupported as 
   horribly intrusive for little gain).

3. Bug detection updated and shuffled -- the multiply/divide+shift problem 
   is lethal enough that if not worked around it makes the kernel crash in 
   time_init() because of a division by zero; the daddiu erratum might 
   also trigger early potentially, though I have not observed it.  On the 
   other hand the daddi detection code requires the exception subsystem to 
   have been initialised (and is there mainly for information).

4. r4k_daddiu_bug() added so that the existence of the erratum can be 
   queried by code at the run time as necessary; useful for generated code 
   like TLB fault and copy/clear page handlers.

5. __udelay() updated as it uses multiplication in inline assembly.

 Note that -mdaddi requires modified toolchain (which has been maintained 
by myself and available from my site for ~4years now -- versions covered 
are GCC 2.95.4 - 4.1.2 and binutils from 2.13 onwards).  The -mfix-r4000 
and -mfix-r4400 have been standard for a while though.

86ef5c9a8e cpu-hotplug: replace lock_cpu_hotplug() with get_online_cpus()
Replace all lock_cpu_hotplug/unlock_cpu_hotplug from the kernel and use
get_online_cpus and put_online_cpus instead as it highlights the
refcount semantics in these operations.

The new API guarantees protection against the cpu-hotplug operation, but
it doesn't guarantee serialized access to any of the local data
structures. Hence the changes needs to be reviewed.

In case of pseries_add_processor/pseries_remove_processor, use
cpu_maps_update_begin()/cpu_maps_update_done() as we're modifying the
cpu_present_map there.

The tc35815-mac platform device used a pci bus number and a devfn to
identify its target device, but the pci bus number may vary if some
bus-bridges are found.  Use irq number which is be unique for embedded
controllers.

0487de9142 [MIPS] Malta: Fix reading the PCI clock frequency on big-endian
The JMPRS register on Malta boards keeps a 32-bit CPU-endian
value. The readw() function assumes that the value it reads is a
little-endian 16-bit number. Therefore, using readw() to obtain
the value of the JMPRS register is a mistake. This error leads
to incorrect reading of the PCI clock frequency on big-endian
during board start-up.

Change readw() to __raw_readl().

This was tested by injecting a call to printk() and verifying
that the value of the jmpr variable was consistent with current
setting of the JP4 "PCI CLK" jumper.

84c21e2542 [MIPS] Malta: Fix software reset on big endian
I noticed that the commit f197465384
(MIPS Tech: Get rid of volatile in core code) broke the software
reset functionality for MIPS Malta boards in big-endian mode.

According to the MIPS Malta board user's manual, writing the magic
32-bit GORESET value into the SOFTRES register initiates board soft
reset. My experimentation has shown that the endianness of the GORESET
integer should thereby be the same as the endianness, which has been
set for the CPU itself. The writew() function used to write the magic
value in the code introduced by the commit mentioned above, however,
swaps bytes for big-endian kernels and transfers 16 bits instead of 32.

The patch below replaces the writew() function by the __raw_writel()
routine, which leaves the byte order intact and transfers the whole
MIPS machine word. Trivial code cleanup (replacing spaces by a tab
and cutting oversized lines to make checkpatch.pl happy) is also
included.

The patch was tested using a Malta evaluation board running in both
BE and LE modes. For both modes, software reset was fully functional
after the change.

P.S. I suspect that the same commit broke the "standby" functionality
for MIPS Atlas boards. However, I did not touch the Atlas code as I
don't have such board at my disposal and also because the linux-mips.org
Web site claims that Atlas support is scheduled for removal.

00a5825332 Fix oprofile configuration breakage
The cleanup 09cadedbdc broke the oprofile
configuration for MIPS by allowing oprofile support to be built for
kernel models where oprofile doesn't have a chance in hell to work.

Just a dependecy list on a number of architectures is - surprise - broken
and should as per past discussions probably in most considered to be
broken in most cases.  So I introduce a dependency for the oprofile
configuration on ARCH_SUPPORTS_OPROFILE.

cce335ae47 [MIPS] 64-bit Sibyte kernels need DMA32.
Sibyte SOCs only have 32-bit PCI.  Due to the sparse use of the address
space only the first 1GB of memory is mapped at physical addresses
below 1GB.  If a system has more than 1GB of memory 32-bit DMA will
not be able to reach all of it.

For now this patch is good enough to keep Sibyte users happy but it seems
eventually something like swiotlb will be needed for Sibyte.

The R4000 and R4400 have an errata where if the cp0 count register is read
in the exact moment when it matches the compare register no interrupt will
be generated.

This bug may be triggered if the cp0 count register is being used as
clocksource and the compare interrupt as clockevent.  So a simple
workaround is to avoid using the compare for both facilities on the
affected CPUs.

This is different from the workaround suggested in the old errata documents;
at some opportunity probably the official version should be implemented
and tested.  Another thing to find out is which processor versions
exactly are affected.  I only have errata documents upto R4400 V3.0
available so for the moment the code treats all R4000 and R4400 as broken.

This is potencially a problem for some machines that have no other decent
clocksource available; this workaround will cause them to fall back to
another clocksource, worst case the "jiffies" source.

This matters to any sort of device that is wired to one of the CPU
interrupt pins on an SMP system.  Typically the scenario is most easily
triggered with the count/compare timer interrupt where the same interrupt
number and thus irq_desc is used on each processor.

In case of the cp0 compare interrupt this means the interrupt will be
acked and not handled or re-armed on CPU b, so there won't be any timer
interrupt until the count register wraps around.

With kernels 2.6.20 ... 2.6.23 we usually were lucky that things were just
working right on VSMP because the count registers are synchronized on
bootup so it takes something that disables interrupts for a long time on
one processor to trigger this one.

For scenarios where an interrupt is multicasted or broadcasted over several
CPUs the existing code was safe and the fix will break it.  There is no
way to know in the interrupt controller code because it is abstracted from
the platform code.  I think we do not have such a setup currently, so this
should be ok.

8dfa741f14 [MIPS] Sibyte: Stop timers before programming next even.
We have no guarantee by the generic time code that the timer is stopped
when the ->next_event method is called.  Modifying the Timer Initial Count
register while the timer is enabled has UNPREDICTABLE effect according to
the BCM1250/BCM1125/BCM1125H User Manual.  So stop the timer before
reprogramming.

This is a paranoia fix; no ill effects have been observed previously.

622477533d [MIPS] Sibyte: Increase minimum oneshot timer interval to two ticks.
For the old minimum of a single tick a value of zero would be programmed
into the init value register which in the BCM1250/BCM1125/BCM1125H User
Manual in the Timer Special Cases section is documented to have
UNPREDICTABLE effect.

Observable sympthoms of this bug were hangs of several seconds on the
console during bootup and later if both dyntick and highres timer options
were activated.

In theory contiguous mode of the timers is also affected but in an act of
hopeless lack of realism I'll assume nobody will ever configure a KERNEL
for HZ > 500kHz but if so I leave that to evolution to sort out.

f6771dbb27 [MIPS] Fix shadow register support.
Shadow register support would not possibly have worked on multicore
systems.  The support code for it was also depending not on MIPS R2 but
VSMP or SMTC kernels even though it makes perfect sense with UP kernels.

SR sets are a scarce resource and the expected usage pattern is that
users actually hardcode the register set numbers in their code.  So fix
the allocator by ditching it.  Move the remaining CPU probe bits into
the generic CPU probe.

The recent switch of the Sibyte SOCs from the processor specific cache
managment code in c-sb1.c to c-r4k.c lost this old hack

    [MIPS] Hack for SB1 cache issues

    Removing flush_icache_page a while ago broke SB1 which was using an empty
    flush_data_cache_page function.  This glues things well enough so a more
    efficient but also more intrusive solution can be found later.

74521c28e5 Use i8253.c lock for PC speaker on MIPS, too.
The Jazz machines have to use the PIT timer for dyntick and highresolution
kernels.  This may break because currently just like i386 used to do MIPS
uses two separate spinlocks in the actual PIT code and the PC speaker
code.  So switch to do it the same that x86 currently does PIT locking.

a76ab5c10d [MIPS] MT: Fix bug in multithreaded kernels.
When GDB writes a breakpoint into address area of inferior process the
kernel needs to invalidate the modified memory in the inferior which
is done by calling flush_cache_page which in turns calls
r4k_flush_cache_page and local_r4k_flush_cache_page for VSMP or SMTC
kernel via r4k_on_each_cpu().

As the VSMP and SMTC SMP kernels for 34K are running on a single shared
caches it is possible to get away without interprocessor function calls.
This optimization is implemented in r4k_on_each_cpu, so
local_r4k_flush_cache_page is only ever called on the local CPU.

This is where the following code in local_r4k_flush_cache_page() strikes:

        /*

        if (cpu_context(smp_processor_id(), mm) == 0)
                return;

On VSMP and SMTC had a function of cpu_context() for each CPU(TC).

So in case another CPU than the CPU executing local_r4k_cache_flush_page
has not accessed the mm but one of the other CPUs has there may be data
to be flushed in the cache yet local_r4k_cache_flush_page will falsely
return leaving the I-cache inconsistent for the breakpoint.

While the issue was discovered with GDB it also exists in
local_r4k_flush_cache_range() and local_r4k_flush_cache().

Fixed by introducing a new function has_valid_asid which on MT kernels
returns true if a mm is active on any processor in the system.

This is relativly expensive since for memory acccesses in that loop
cache misses have to be assumed but it seems the most viable solution
for 2.6.23 and older -stable kernels.

21b2aecaae [MIPS] c-r3k: Implement flush_cache_range()
Contrary to the belief of some, the R3000 and related processors did have
caches, both a data and an instruction cache.  Here is an implementation
of r3k_flush_cache_page(), which is the processor-specific back-end for
flush_cache_range(), done according to the spec in
Documentation/cachetlb.txt.

While at it, remove an unused local function: get_phys_page(), do some
trivial formatting fixes and modernise debugging facilities.

098362e720 [MIPS] Add len and addr validation for MAP_FIXED mappings.
Mmap with MAP_FIXED was not validating the addr and len parameters.  This
leads to the failure of GCC's gcc.c-torture/execute/loop-2[fg].c testcases
when using the o32 ABI on a 64 bit kernel.

These testcases try to mmap 65536 bytes at 0x7fff8000 and then access all
the memory.  In 2.6.18 and 2.6.23.1 (and likely other versions as well)
the kernel maps the requested memory, but since half of it is above
0x80000000 a SIGBUS is generated when it is accessed.

This patch moves the len validation above the MAP_FIXED processing so that
it is always validated.  It also adds validation to the addr parameter for
MAP_FIXED mappings.

Convert jmr3927_clock_event_device to more generic
txx9tmr_clock_event_device which supports one-shot mode.  The
txx9tmr_clock_event_device can be used for TX49 too if the cp0 timer
interrupt was not available.

Convert jmr3927_hpt_read to txx9_clocksource driver which does not
depends jiffies anymore.  The txx9_clocksource itself can be used for
TX49, but normally TX49 uses higher precision clocksource_mips.
