Explicitly provide the first internal volume ID value in the comment for
UBI_INTERNAL_VOL_START. This allows developers who, when adding features
related to volume ids and observe unexpected very large volume ids, to grep
for the observed value in the source code and find out immediately that it is
expected behaviour.

This patch amends commentaries in scan.[ch] to match the new logic. Reminder -
we did the restructuring to prepare the code for adding the fastmap. This patch
also renames a couple of functions - it was too difficult to separate out that
change and I decided that it is not too bad to have it in the same patch with
commentaries changes.

There was an attempt to standartize various "__attribute__" and
other macros in order to have potentially portable and more
consistent code, see commit 82ddcb0405.
Note, that commit refers Rober Love's blog post, but the URL
is broken, the valid URL is:
http://blog.rlove.org/2005/10/with-little-help-from-your-compiler.html
Moreover, nowadays checkpatch.pl warns about using
__attribute__((packed)):
"WARNING: __packed is preferred over __attribute__((packed))"
It is not a big deal for UBI to use __packed, so let's do it.

An image sequence number is added to the UBI erase-counter header
to be able determine if the root file system contains a mixture
of old and new images (because the flashing failed to complete).
A change to nolo is also needed for this to take effect.

Before UBI got into mainline, there was a slight flash format
change - we did not have sequence number support, then added it.
We have carried full support of those ancient images till this
moment. Now the support is removed, well, not fully removed.
Now UBI will support only _clean_ old images, which were cleanly
detached last time (just before kernel upgrade). This is most
likely the case.
But we will not support unclean ancient images. Surprisingly,
this allows us to remove a big chunk of legacy code.
And the same should be true for downgrading: clean images should
downgrade fine, but unclean ones will not.

The new trend in linux is not to store headers which define
on-media format in the include/ directory, but instead, store
them locally. This is because these headers "do not define any
kernel<->userspace interface".
Do so for UBI as well.
